1. Minimum commitment: 1 month
2. Your Prevent Plan will start from the date of payment.
3. Direct debit payments will continue past the minimum commitment date until cancelled by the client.
4. All Prevent Plan cancellations must be made in writing to contact@physiofocused.com
5. All monthly service inclusions are valid for 1 month only and services will reset at the beginning of the new month term.
6. Any services not used within the month term cannot be rolled over into the next month.
7. Partnerships with gyms, personal trainers, and other healthcare providers may change during the time period.
8. If you are using an itison voucher your card will only be charged when your 'free trial' ends 30 days from sign up.
